If it is a newer model with touch screen, that setting is on the Now Playing screen. It is a button that looks like two arrows in a loop. If you see a "1" on the button, it is set to repeat current song. Tap it to change to Off or All.

I suspect that you have the repeat 1 song option set. Tap that icon once more to turn it off. Then shuffle should work as expected. There could be some software issue on the ipod which is disabling those dots If it is a click wheel ipod, the repeat option is in settings > repeat If this happens again, I would suggest a restore of the ipod.
